Cerbo alternatives for longevity clinics in 2026

A line from Cerbo, a dim point, to a bright point that branches into Bookings, Programmes and Follow-up, on a sand background
Neogenic is our product. We make it, we wrote this comparison, and Neogenic is listed first. Facts about other vendors come from their own websites, checked on 26 October 2026, and may have changed since.

Cerbo is an EHR for functional medicine, direct primary care and cash-based practices.[1] It covers charting, scheduling, subscription billing, a configurable portal and e-prescribing, and its site says it has helped more than 5,000 practitioners.[1][2]

Whether you should leave depends on what you use it for. A clinic that charts all day needs a different alternative from a clinic that mostly sells and runs programmes.

Start with what Cerbo does for you

  • If Cerbo is your clinical record and prescribing tool, look at other EHRs, and ask each one about e-prescribing in your country.
  • If Cerbo mostly runs bookings, subscriptions and the portal, a clinic operations platform may fit better and cost less per seat.
  • If you are in the EU, the data location may decide it. A Cerbo article says its data is housed in US data centres.[3]

The real monthly cost of Cerbo

Cerbo prices per user.[2] A full-time prescribing provider is $281 a month, a non-prescribing provider $256, a supporting provider $127 and each extra staff member $63. Part-time seats cost less.[2]

Add-ons come next. AI Scribe is $29 per provider per month for up to 60 notes, the patient portal starts at $79 per month for up to 1,000 patients, and setup is a one-time $1,195, or $595 for a new practice.[2]

Take 1 prescribing provider, 1 non-prescribing provider and 3 staff, all full-time, with the portal and AI Scribe for both providers. That is $281 + $256 + 3 × $63 + $79 + 2 × $29 = $863 a month, plus setup. Use the same team for every quote you collect.

How we compared

We used only each vendor's own pages and gave no scores. Our four questions follow the reasons clinics give for leaving Cerbo.

  1. Is it an EHR, an operations platform, or both?
  2. Where does the vendor say clinic data is stored?
  3. How does the price grow with each person who signs in?
  4. Does it have AI, and is it extra?

1. Neogenic

Neogenic covers the work around the clinical record. Neo handles bookings, contacts, tasks and follow-up when your team asks in plain words, within each person's permissions, and it asks before changes that are hard to undo. Billing covers memberships, deposits and instalments, and the patient portal runs on the web, with iOS and Android coming soon.

Clinic records are stored in the EU. The GDPR page names Stockholm for databases and files and Frankfurt for the application. A few supporting services, such as email delivery, use providers outside the EU, under the EU Standard Contractual Clauses. The longevity clinic software overview shows each area.

Watch out if you rely on Cerbo's EHR. Neogenic has no e-prescribing, superbills or insurance claims. It is also new, with few reference clinics so far.

2. OptiMantra

OptiMantra is lighter and cheaper to start, at $99 per month for the first practitioner, with clerical staff free.[6] It has charting, labs and telehealth for integrative practices.[13]

Watch out: one company is behind both. OptiMantra's privacy policy and subscription agreement are issued by CerboOM, LLC, whose policy also covers cer.bo.[14] Moving from Cerbo to OptiMantra may keep you with the same company.

3. Healthie

Healthie has an EHR, scheduling, billing, a client portal, telehealth and programmes[7], with plans from $18 per month billed yearly.[9]

Watch out for the same US question. Healthie stores data in the United States.[8] Its AI charting is a paid add-on.[9]

4. Reya.ai

Reya.ai is built for longevity operators, with AI agents for food, assessments, health monitoring, labs, scribe notes and reports.[15] Essentials is $850 per month and Professional starts at $1,800.[11]

Watch out for the timeline and the region. Its FAQ says most clinics go live within 4 to 16 weeks, and the pages we checked name no hosting region.[10] Its Advanced plan lists multi-cloud and domiciling options.[11]

5. LongevAI

LongevAI reads lab reports onto the clinic's own reference ranges and drafts plans a clinician approves, with a white-label client portal.[12][16]

Watch out for the missing price list. LongevAI shares its price in a demo, and it takes client payments through Stripe rather than describing invoices.[12]

When Cerbo is still the better choice

Stay with Cerbo if your clinicians chart and prescribe in it every day and you are a US practice. A full EHR with e-prescribing is hard to replace with a tool that is not an EHR.

A plan for the move

  1. Ask Cerbo which exports it offers for patients, charts, subscriptions and open balances.
  2. Decide which system keeps the clinical record from now on.
  3. Move bookings, subscriptions and follow-up first, then switch patients to the new portal.
  4. Keep Cerbo read-only until every open subscription has billed once in the new tool.

What Neogenic costs

Exploration is €270 ($299) per person per month, for up to 4 people. ClinicOS starts at €1,320 ($1,499) per location per month with 6 staff accounts. A one-time setup fee of €900 ($990) is free on the yearly plan, and yearly billing gives 2 months free. Prices exclude VAT. See pricing for add-ons.

Frequently asked questions

How much does Cerbo cost per month?

Cerbo prices per user. At full-time rates a prescribing provider is $281, a non-prescribing provider $256, a supporting provider $127 and extra staff $63 per month. Setup is a one-time $1,195, or $595 for a new practice.

Where does Cerbo store data?

Cerbo runs each practice on its own dedicated cloud server, and a Cerbo article says its data is housed in US data centres. We found no EU hosting option on its site.

Can Neogenic replace Cerbo?

It can replace the operations side, such as booking, follow-up, memberships, invoices and the patient portal. It does not replace an EHR with e-prescribing.

Are Cerbo and OptiMantra the same company?

The privacy policy on optimantra.com is issued by CerboOM, LLC and covers both cer.bo and optimantra.com. Ask the vendor how the two products will develop.
