OptiMantra alternatives for longevity clinics in 2026
OptiMantra is practice software for integrative medicine and wellness practitioners.[1] It lists online booking, reminders, charting, telehealth, POS, labs and a patient portal[2], and it supports recurring memberships and prepaid packages for cash-based practices.[1]
It is priced for a small practice. Clinics usually look elsewhere for one of three reasons. The team has grown, the clinic is in Europe and needs a stated data region, or it wants AI that does more than write notes.
Who runs OptiMantra now
OptiMantra's privacy policy and subscription agreement are issued by CerboOM, LLC.[3][4] The privacy policy covers both optimantra.com and cer.bo, the site of the Cerbo EHR.[3] If Cerbo is on your list as the way out of OptiMantra, ask both teams whether you would be changing vendor at all.
What OptiMantra costs as the team grows
OptiMantra charges $99 per month for the first practitioner, $49 for each extra practitioner and $25 for each clinical staff member, and clerical staff are free.[5]
Take a practice with 2 practitioners and 3 clinical staff. At list price it pays $99 + $49 + 3 × $25 = $223 a month before add-ons. That is low. The real question is what the clinic needs next.
AI Charting is an optional add-on through a partner, DeepCura.[6] The pricing page shows no price for it, and e-prescribing and insurance claims are paid add-ons too.[5]
The EU questions to ask
- Where is the platform hosted? OptiMantra says it runs on AWS[6], and its FAQ names a security standard for its data centre but no country.[1]
- Where does personal data go? Its privacy policy says data given through its website is processed in the United States, and it uses Standard Contractual Clauses for transfers from the EU and UK.[3]
- Is there a Data Processing Agreement that names every subprocessor? Ask for it before you sign.
- Can you bring your own AI tools? Its subscription agreement bars AI agents and automated tools unless CerboOM allows them in writing or they are features inside the service.[4]
How we compared
We read each vendor's own site and gave no scores. For a practice leaving OptiMantra we asked four things.
- Is it a different company from OptiMantra?
- Does the site say where clinic data is stored?
- Is there AI beyond note writing, and does it cost extra?
- How does the price grow from 2 people to 10?
Five alternatives at a glance
| Tool | Data location stated | AI | Pricing unit |
|---|---|---|---|
| Neogenic | EU. Stockholm and Frankfurt | Neo, an AI agent for clinic admin, with a monthly allowance on each plan | Per person, or per location |
| Cerbo | US data centres[7] | AI Scribe, $29 per provider for up to 60 notes a month[8] | Per user, plus a setup fee |
| Healthie | United States[9] | AI charting as a paid add-on[10] | Per plan, by active clients |
| Semble | UK and EU data centres[11] | AI-assisted notes[12] | Quote after a demo[13] |
| LongevAI | In the clinic's region[14] | Vita assistant, and AI-drafted reports a clinician approves[15] | Quote after a demo |
1. Neogenic
Neogenic changes the model, not only the vendor. Neo, its AI agent, does admin work for your team. It books and moves appointments, updates contacts, creates tasks and drafts follow-up, and it waits for approval before changes that are hard to undo.
Billing covers the programme side OptiMantra users know, with memberships, subscriptions, deposits and instalments in Billing & Payments. Clinic records are stored in the EU, with databases and files in Stockholm and the application in Frankfurt, as the GDPR page sets out. A few supporting services, such as email delivery, use providers outside the EU, under the EU Standard Contractual Clauses. See how the platform covers a longevity clinic.
Watch out for the clinical side. Neogenic has no e-prescribing or insurance claims, which OptiMantra offers as add-ons.[5] It is also new and in English only.
2. Cerbo
Cerbo is a full EHR for functional, direct primary care and cash-based practices, with e-prescribing and subscription billing.[16][8] One company, CerboOM, LLC, issues the privacy policy for both cer.bo and optimantra.com, so a move between them may be a move within one company.[3]
Watch out for the bill. Full-time providers cost $281, $256 or $127 a month, extra staff $63 each, and setup is $1,195.[8] Its data sits in US data centres.[7]
3. Healthie
Healthie is a broad US practice platform with an EHR, programmes and workflow automations, and its site cites more than 40,000 providers.[17][18] Its Core plan starts at $18 a month billed yearly, for 10 active clients.[10]
Watch out for the data location. Healthie stores data in the United States under the EU-US Data Privacy Framework.[9]
4. Semble
Semble is a platform for private healthcare, based in London, with an EHR, Semble Pay and a patient portal.[12][19] Its security page says data is held in UK and EU data centres.[11] Its help centre describes membership schemes billed monthly or yearly.[20]
Watch out for the price. Semble publishes none, so it only comes up in a demo.[13]
5. LongevAI
LongevAI, from Leiden in the Netherlands[21], builds LongevOS for longevity clinics. It reads lab reports onto the clinic's own reference ranges and drafts reports and plans that a clinician approves.[14][15]
Watch out for billing. Its site describes client payments through Stripe, not invoicing.[14]
When OptiMantra is still the better choice
Stay if you are a small US integrative practice and the price per practitioner suits you. Charting, labs and telehealth in one low-cost tool are hard to beat at that size.
Moving off OptiMantra
- Ask OptiMantra which exports it offers for patients, upcoming appointments and open invoices.
- List every active membership and prepaid package with its balance and renewal date.
- Decide where clinical charts will live if your new tool is not an EHR.
- Run both systems for a short overlap, with OptiMantra read-only.
